What happens after you set-up an appointment at our clinic
Once your appointment has been made, our staff can email you the necessary forms to complete prior to your appointment to avoid wait time. Please complete the forms required prior to your appointment so our staff can process the forms and Dr. Balcos is able to review your Patient Assessment form to provide you the treatment needed during your initial visit.
If you are unable to complete the form in advance, please plan to arrive at least 15 minutes prior to appointment schedule.
You may print the Patient Intake Form to submit any demographic changes and health insurance update to our office. The Patient Intake Form can be sent via fax to (714) 948-8248 or by email at firstname.lastname@example.org.
